When you place the tablet on your tongue, it dissolves and is swallowed with saliva. The zolmitriptan is absorbed into the bloodstream, reaches the brain, and binds to 5-HT₁B/1D receptors. This causes:
constriction of dilated blood vessels in the brain’s meninges, reducing headache pain
inhibition of neuropeptide release (such as CGRP) that contribute to pain and inflammation
reduced transmission of pain signals in the trigeminal nerve pathway As a result, the intensity of migraine symptoms (headache, nausea, light/sound sensitivity) is lessened, and relief may be achieved more quickly due to the fast-dissolving formulation.
Zomig Rapimelt is indicated for the acute treatment of migraine attacks with or without aura in adults. It is not intended for:
use to prevent migraine attacks
relief of other types of headache (such as tension-type headache)
use in children, or adolescents under 18 years (efficacy was not demonstrated)
regular daily use to control recurring headaches beyond medical advice
Take Zomig Rapimelt as soon as you notice the first signs of a migraine attack.
Place one tablet on your tongue, allow it to dissolve, then swallow with saliva; you do not need a drink of water.
If symptoms persist or return within 24 hours, a second dose may be taken only after at least 2 hours from the first dose (for the 2.5 mg strength). For patients using 5 mg, your doctor will advise accordingly.
Do not exceed 10 mg in 24 hours.
Not recommended for patients with moderate to severe hepatic impairment beyond specified limits — your doctor may limit your single dose to 5 mg in 24 h.
Do not use for children under 18 years or as a preventative migraine therapy.
Do not take a dose of Zomig if your headache is not clearly diagnosed as migraine, or if you have risk factors for cardiovascular disease unless approved by your doctor.
Store at room temperature, not above 25 °C, in the original blister pack until ready to use. Keep out of reach of children. Do not use after the expiry date printed on the pack. The blister must be peeled—not pushed through. Disposal should be in accordance with local requirements; do not throw into wastewater or household waste.
Common side effects (may affect up to 1 in 10 people) include:
Tingling, altered sensation in fingers/toes/skin
Feeling sleepy, dizzy or warm
Nausea, dry mouth
Muscle weakness or mild chest, neck or limb heaviness/pressure
Uncommon/rare but serious side effects:
Fast or irregular heartbeat
Chest pain or signs of heart attack
Stroke symptoms (sudden numbness, speech difficulty, vision changes)
Allergic reactions (swelling, rash, difficulty breathing)
Medication-overuse headache (if used too frequently) If you experience any serious symptoms, stop using Zomig Rapimelt and contact your doctor or seek emergency help immediately.
Do not use Zomig Rapimelt if you have uncontrolled high blood pressure, ischaemic heart disease, coronary vasospasm (Prinzmetal’s angina), previous heart attack or stroke (TIA), or severe hepatic impairment.
Use cautiously if you have risk factors for cardiovascular disease (e.g., diabetes, high cholesterol, smoking, family history).
If your headaches change in pattern, worsen or start to occur more frequently (daily or nearly daily), consult your doctor — you may be developing chronic migraine or medication over-use headache.
Zomig Rapimelt may affect your ability to drive or use machines — the drug or the migraine attack itself may cause tiredness or dizziness—wait until you feel fully alert.
Inform your doctor if you are pregnant, planning to become pregnant, or breastfeeding. Limited data exist — use only if clearly needed.
Avoid combining with ergotamine-type drugs or other triptans within 24 hours. Also avoid St. John’s Wort and strong inhibitors/inducers of CYP1A2 without notification.
Frequent use for headache relief may worsen your headaches — follow your doctor’s plan and avoid using too often.
Zomig Rapimelt interacts with:
Other triptans or ergotamine derivatives (increased risk of vasoconstriction) — at least 24 hours between doses required.
MAO-A inhibitors (e.g., moclobemide) — increased exposure; maximum 5 mg/24 h.
Strong CYP1A2 inhibitors (e.g., fluvoxamine) or CYP3A4 inhibitors — may increase zolmitriptan levels.
Herbal remedies like St. John’s Wort (hypericum) — may trigger serotonin syndrome when combined.
Other serotonergic medications (SSRIs, SNRIs, tramadol) — risk of serotonin syndrome. Always provide full medication list and supplement use to your doctor or pharmacist.
Symptoms following overdose may include sedation, unconsciousness or cardiovascular symptoms (heart rate changes, seizures). There is no specific antidote — treatment is supportive and monitoring is required, including cardiovascular monitoring for at least 15 hours post-ingestion.
