The active ingredient is heparin sodium. Heparin enhances the effect of antithrombin III, which inactivates thrombin and factor Xa, thereby reducing formation of fibrin clots. This action helps prevent the growth or extension of existing clots and formation of new ones.
Dosage is individualized and depends on the indication, patient weight, clotting risk, and lab monitoring (aPTT or anti-Xa levels).
Intravenous infusion: typically begins with a loading dose followed by continuous infusion.
Subcutaneous injections: may be used for prophylaxis (e.g. in surgical patients).
Use caution when switching between strengths.
Vials should be handled under sterile conditions. Use only as directed by medical staff.
Do not mix with other medications unless compatibility has been confirmed.
Store Heparin vials at room temperature, away from light. Do not freeze. Keep vials in their original packaging until use. Do not use if solution is discolored or contains particles.

Hypersensitivity reaction

Adjust dosage in kidney or liver impairment, though heparin is less dependent on renal clearance
Be cautious in pregnant women (heparin does not cross the placenta)
Avoid use concurrently with other anticoagulants unless instructed
Heparin interacts with many agents that increase bleeding risk, including:
Antiplatelet drugs (e.g. aspirin, clopidogrel)
Thrombolytics
NSAIDs
